Application Procedures

The DS-156 Electronic Visa Application Form (EVAF) must be completed online, and can be accessed at http://evisaforms.state.gov
You can download forms DS-157 (Supplemental Nonimmigrant Visa Application) and DS-158 (Contact Information And Work History for Nonimmigrant Visa Applicant) here in both English and German.

Who has to submit which forms?

• Form DS-156 (Nonimmigrant Visa Application) has to be submitted by all nonimmigrant visa applicants.
• Form DS-157 (Supplemental Nonimmigrant Visa Application) has to be submitted by all male nonimmigrant visa applicants between the ages of 16 and 45, regardless of nationality and regardless of where they apply.
• Form DS-158 (Contact Information And Work History for Nonimmigrant Visa Applicant) has to be submitted by all applicants seeking F, J and M visas.

The application forms should be brought along with the necessary documentation to the consular section in Berlin, Frankfurt or Munich.

If you need more space to answer any question, use the bottom half of the back of the application form, making sure that all answers are clearly numbered. Please leave the top half free for our office use.

Note: All items on the Form DS-156, and on any supporting forms (Form DS-157, Supplemental Nonimmigrant Visa Application, and/or Form DS-158, Contact Information and Work History for Nonimmigrant Visa Applicant) must be completed.

Form DS-156

To be completed online by all nonimmigrant visa applicants. Please note that these are electronic application forms. Please read the instructions carefully.

Important Technical Note:
What you need:
• Your Internet browser must support 128-bit encryption.
• If you are using Internet Explorer (Windows), the minimum version that will work with this site is version 5.0, with service pack 2.
• If you are using Netscape, the minimum version that will work with this site is version 6.2.
• You must also have Adobe Acrobat Reader version 5.0 or higher installed on your PC in order to download and print the completed application form. If you do not have Adobe Acrobat Reader version 5.0 or higher, you may download a copy from Adobe's web site.
• You should use either an ink-jet or laser printer to print the completed application form.

Note: Depending on the speed of your Internet connection, it may take several minutes to download the initial and completed copies of the form. Please be patient.

Form DS-157

To be submitted by all male nonimmigrant visa applicants between the ages of 16 and 45, regardless of nationality and regardless of where they apply.
